AB 685 Template Notices

Please find attached updated AB 685 Template Notices for use in connection with workplace exposures or outbreaks of COVID-19. These templates are intended to replace the previous version provided by our office on December 7, 2020.

The templates have been refined and tailored to provide employees with more specific information around their individual exposure while at a workplace, and to enhance the level of detail an employer provides to the local health department when reporting an outbreak at a workplace.  The Notice to Health Department of Workplace Outbreak now allows the employer to specify both the physical business address of workplace, as well as an individual employee’s specific duty location within a workplace.
